Consent forms
Read them before you book. They are signed at the studio, on the day of your appointment.
When you book I ask you to confirm that you read this. The signing happens in person, with you right there, so you can ask me anything before we start. If something is not clear, message me and we talk it through.
What I am going to ask you
On the day of your appointment we fill out your chart together: name, date of birth, phone and email, the condition of your skin, how it heals, how you care for it at home and whether you wear contact lenses. Your data is confidential. It is not sold, not shared and not resold.
Micropigmentation
You confirm that the information you give is true. You acknowledge that reactions may show up, such as redness, burning, itching, loss of pigment, swelling, allergic reactions or other responses particular to your body, and you agree to follow the aftercare instructions to avoid infection and get a better result.
You understand that micropigmentation takes several sessions and that the touch up is scheduled between 30 and 45 days after the procedure. The result is not evaluated until the touch ups are done.
Aftercare for your micropigmentation
- Do not touch, rub or scratch the treated area, and do not handle it with your fingers.
- Do not put makeup on the area that was just treated.
- No pool, ocean, sauna or jacuzzi while it heals.
- Stay out of the sun and keep soap off the area.
- The area has to heal, flake and lighten. The process can take up to 10 days and the color looks darker at the beginning.
- If you donate blood, the Red Cross advises waiting 6 months after permanent makeup.
- If you have itching, swelling, burning or any symptom that worries you, let me know right away.
Facial treatments
- Nature of the service. Facials aim to improve the appearance and the condition of your skin. Results vary with your skin type, your history and your aftercare.
- Medical conditions and allergies. You confirm that you have truthfully disclosed any allergy, skin condition, sensitivity, topical or oral medication, medical treatment or previous aesthetic procedure that could affect the service.
- Possible risks. You acknowledge that there can be temporary effects such as redness, mild burning, flaking, sensitivity, breakouts or unexpected reactions.
- Products. You authorize the use of professionally approved products, understanding that every skin reacts differently.
- Results. No specific result is guaranteed. Results depend on your care at home and on following the instructions.
- Aftercare. You agree to stay out of the sun, not to pick at your skin and to use the recommended products.
- Release of liability. You release the professional from liability for reactions that come from information you left incomplete or left out.
- Authorization. You voluntarily consent to receive the facial treatment you requested.

Minors
The services are for people 18 and over. If you are a minor, you need the authorization and the presence of your mother, father or guardian.
